Help for MNHC patients
MyChart is a secure website and app for patients who already receive care at MNHC. Use it to view test results, request medication refills, and manage appointments.

If you already receive care here, call MNHC for an activation code.
Do not use MyChart. Call to become a patient or request your first appointment.

About MyChart
MyChart is the secure patient portal used by MNHC. It lets you view parts of your health record and complete routine tasks from a website or mobile app.
MNHC uses MyChart with support from OCHIN, a nationwide network of community health centers, clinics, hospitals, and other care organizations. When another organization participates, MyChart may let you link accounts and see allergies, medications, visits, and results in one place. What appears depends on each organization.
