New patients welcome — we accept Medi-Cal, Medicare, Healthy San Francisco, and most commercial insurance, and our team can help you enroll or stay enrolled. Sliding-scale fees regardless of ability to pay. The fastest way to start is by phone — Mon–Fri 8am–5pm.
As a Federally Qualified Health Center, MNHC offers sliding-scale fees for every patient — insured or not, documented or not. Eligibility is based only on household size and income. See the Sliding Fee Discount Program →
Call (415) 552-3870 Monday through Friday, 8am to 5pm. Bilingual English/Spanish staff are available. If your primary language is neither, tell us what you speak — we provide free interpreters in 15+ languages.
Intake staff will ask for your name, date of birth, address or neighborhood, and a contact phone number. If you have insurance, have the card handy. If you don't, no worries — we'll help you get enrolled.
We'll match you with a provider and find a time that works. New-patient appointments for pediatrics, adult medicine, women's health, and teen services typically schedule within 1–2 weeks depending on demand.
Arrive 15 minutes early to finish enrollment paperwork and the sliding-fee application if you need one. Bring whatever you can from the list below — but if you have none of these documents, we can still see you.

Photo ID
Driver's license, passport, consular ID, or SF Municipal ID — any works.
Proof of income (past 30 days)
Pay stubs, Social Security/unemployment letter, self-employment ledger, or a signed self-attestation form.
Proof of household size
Tax return or a signed declaration listing dependents.
Insurance card (if you have one)
We accept Medi-Cal, Medicare, Healthy SF, and most commercial insurance.
