Quality Performance Corner

MNHC Clinical Outcomes Beat U.S. Standards

To ensure that we deliver consistent quality of care for all of our patients. MNHC monitors clinical care and patient/family experience in a variety of ways.  Assessing our progress is a foundation of our service excellence goals.

MNHC uses as benchmarks the Healthcare Effectiveness Data and Information Set (HEDIS) created by the National Center for Quality Accreditation to monitor its clinical outcomes. HEDIS is a tool used by more than 90 percent of America’s health plans and medical providers to measure performance on important dimensions of care and service making it possible for an “apples to apples” comparison to assess progress against national quality standards.

Others benchmarks are culled from national data from Healthy People 2020, the Centers for Disease Control and the Uniform Data System (UDS), a core set of information appropriate for reviewing the operation and performance of health centers. The measures are mandated by the Health Resources and Services Administration- Bureau of Primary Health Care (HRSA-BPHC)for community health center program grantees.

Here are some of our recent outcomes.

Diabetes (2013)

92% of MNHC active patients with diabetes aged 18 to 75 years had their Hemoglobin A1c test in the past 12 months exceeding the national HEDIS 90% percentile:  >91%.

Pediatrics (2013)

92% of three- year old patients were up to date on their childhood immunizations surpassing the HEDIS 90% percentile of 83%.
