Fact Sheet Mission Bernal Transition

Q.  When is the transition?
A.  Mission Bernal Adult and Pediatric Clinics will transition over to MNHC effective April 2, 2021. The clinics will be closed April 2nd. However, if you have a medical concern you can reach a member of our clinical team by calling (415) 852-4080 or (415) 552-3870. We will open our doors to patients Monday, April 5th.

Q.  Will the Pediatric After-hours clinic be open over the weekend of April 3rd?
A.  The Pediatric After-hours clinic will have a limited schedule the evening of Friday April 2nd 5-8pm and over the weekend from 9am-6pm – note that these hours may vary, please call to confirm: (866) 961-8588.

Q.  Will the Pediatric After-hours clinic have the same phone number?
A.  The phone number will remain the same: (866) 961-8588

Q.  Can I still come to Mission Bernal after the transition?
A.  Yes, Mission Bernal Pediatrics and Adult Clinics will remain at 1580 Valencia St. Suites 506 and 701.

Q.  Will my insurance be accepted?
A.  MNHC is currently working on contracting with existing insurance providers at Mission Bernal. Your upcoming appointment will be honored.

Q.  Can I continue to see the same medical provider?
A.  Yes, the providers will remain the same.

Q.  Will I be able to come in for a future visit I have scheduled?
A.  Yes, your future scheduled visit will be honored and will be confirmed.

Q.  If I am uninsured can I come to the Mission Bernal clinics?
A.  We welcome patients who are uninsured and low-income and offer affordable healthcare on a sliding fee scale based on the patient’s income and family size. At MNHC, no one is denied care due to inability to pay. Mission Neighborhood enrolls patients in access programs including: Healthy San Francisco, Family Pact and Every Woman Counts. We also provide enrollment assistance for Medi-Cal and the Child Health Disability Program (CHDP).

Q.  Will I still be able to use My Health Online?
A.  MNHC does not have a patient portal at this time. We will not be using My Health Online for appointment scheduling or for provider communication. You will still have access to all the information you currently have on My Health Online. You will continue to be able to use My Health Online for any services you access through Sutter such as lab and imaging.

Saturday & Sunday 9am-1:00pm

Q.  What are the clinic hours for Pediatrics After Hours “Sick Kid” clinic”?
A.  Hours for After Hours “Sick Kid” clinic
Monday-Friday 4:30pm-8:30pm
Saturday & Sunday 1pm-6pm

Q.  Whom do I call if I need to reach my doctor after normal clinic hours?
A.  Patients will call the new main number (415) 852-4080 to reach a provider after hours.
The answering service will connect you to the provider.

Q.  Who should I call if I have more questions?
A.  Starting April 2nd, please feel free to contact MNHC staff at (415) 852-4080 with questions or concerns. Prior to this date, you can reach out to your current provider at Mission Bernal clinics.
