FAMILY PLANNING COUNSELOR
GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES
The Family Planning Counselor is primarily responsible for offering family planning counseling and education services to the patients of Mission Neighborhood Health Center.
SAMPLE JOB DUTIES
- Responsible for counseling and educating patients on family planning, pregnancy, human sexuality, and women's health care issues.
- Responsible for completion of Presumptive Eligibility pregnancy test.
- Process billing in accordance with procedures to comply with Family Planning standards and grant reporting requirements.
- Provide information and referral assistance to patients regarding available community resources.
- Participate in training of staff on relevant women's health issues.
- Work closely with Perinatal Program in conducting Family Planning Options counseling.
- Ensure pregnant women are linked to prenatal care services, schedules patients, remind patients of appointments and provide follow-up on missed appointments.
- Participate and conduct health education training for patients of Women’s Services.
QUALIFICATIONS
- Degree in Social Work, Nursing, Counseling, or related health field. Experience may be substituted for up to two years' formal education.
- Knowledge of Options Counseling related to pregnancy testing.
- Bilingual English/Spanish required.
- Two years experience as a Family Planning counselor.
- Good counseling skills required.
- Good computer skills and ability to access information using Internet.
This is an SEIU/UHW, West position
